Agape Healthcare Services will provide 24/7 daily support to persons with intellectual and developmental disabilities (IDD). Agape Healthcare Services will provide IRSS services to individuals wanting to live as independently as possible in the community of their choice.
Community Connector (CC)
Agape Healthcare Services will provide CC services to individuals in the community. CC services are designed to support individuals with opportunities to build relationships and natural supports in the community, interact with community members, promote safety and independence, and work on personal goals. Supported Community Connections (SCC)
Agape Healthcare Services will provide SCC services to individuals in the community. SCC services are designed to support individuals with opportunities to build relationships and natural supports in the community, interact with community members, promote safety and independence, and work on personal goals.
Homemaker Basic
Agape Healthcare Services will provide Homemaker-Basic services designed to support persons with intellectual and developmental disabilities who require assistance to manage basic household tasks necessary to live as independently as possible in the community of their choice.
Homemaker-Enhanced
Agape Healthcare Services will provide Homemaker-Enhanced services that allow the person with an intellectual and developmental disability to receive support with household tasks necessary for the person to remain healthy and safe in their home. Enhanced Homemaker Services consist of the same household tasks as
Mentorship
Agape Healthcare Services will provide Mentorship services to individuals living in the community. Mentorship services are designed to support individuals to develop self-advocacy skills. Service delivery may include providing exposure to community environments and experiences, interviewing potential providers
Non-Medical Transportation
Agape Healthcare Services will provide Non-Medical Transportation services to individuals living in the community.
